Authorization Form

Call us at 913-221-9109 to talk about where you would like to release the ashes so we can determine what permission or permits we will have to obtain and if there might be any restrictions.

The authorization form needs to be filled out and signed. Please make sure to let us know if you have any special requests such as an evening scattering, high altitude release, or a special date you would like us to do our best to perform the memorial on.
